When you come for your appointments, please bring the following:

  • Driver’s license (or valid picture ID)
  • Valid health insurance card(s)
  • Referral Letter or Insurance Authorization (if required)
  • Payment (please read our Insurance & Fees Policy below)
  • A complete and accurate list of your current medications
  • Any tests (x-rays, CT scans, MRIs, EMG, etc.) relevant to your problem
  • Completed Patient Forms
  • Your time and patience. We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.

To cancel an appointment:

Telephone the office during business hours and allow at least 48 hours’ notice so that we can offer your appointment to patients on our waiting list.

Insurance & Fees

Our providers are committed to the success of your medical treatment and care. Please remember that payment of your bill is part of your treatment and care.

Our office is contracted with many Preferred Provider Organization (PPO) insurance plans. However, there are some plans that we are out-of-network with. We urge you to contact your insurance company and ask if you have access to health care services at Orthopaedic Specialty Institute, and what (if any) co-payments, co-insurances, and deductibles will be your responsibility. Co-payments are due at the time of your appointment.
