Help with Disability Benefits

OSI is ready to help you fill out forms

Your OSI provider will describe the road to recovery, so you know how to plan. We understand that your injury and recovery may limit or restrict some of your activities for the short term. These limitations and restrictions could affect your work (e.g., ability to earn income), school, driving, vacation plans, etc.

You will be able to use the provider’s recovery directions so you can then organize what you need to do for your work and disability benefits.

Please understand that all our orthopaedic providers have the expertise to make different types of disability benefits decision(s). Your provider will consider the following factors:

  • Your age
  • Your profession (your job duties)
  • The type of injury you have had
  • The prescribed treatment

What’s required?

For you to apply for disability benefits (e.g., income support, job protection, school excusal, handicap placard, etc.), your provider must authorize that you indeed need to limit or restrict one or more activities. The best time for you to discuss your disability authorization request(s) is during your office appointment.

Work

If your provider approves the disability request, your provider will give you a note that you can use to submit to your employer or your insurance company. This note can often get the disability benefit(s) process started. Please understand that applying for income and job disability benefits takes time and involves different groups which each have their own time frames to complete their step(s).

Jury Duty

Know that you can reschedule your jury duty up to two times without a doctor’s certification. If, because of your injury, you need to reschedule more than these two times, bring this form(s) to your office visit and submit at check out.

Handicap Placard with DMV

Our office has the DMV form. You can request form completion after your office visit at check out. It will take approximately 20 minutes to complete.

EDD Benefits

First Time?

  1. Go to www.edd.ca.gov to register.
  2. Fill out your section. Once completed, you will receive a receipt number.
  3. Contact our office with your receipt number so we can submit our section online.

In order to be eligible for State Disability benefits, you must be off work for at least eight days; there is a seven-day, non-payable waiting period. Please note that EDD Benefits are coordinated by the State of California. OSI does not have the ability to speed up the process, get your benefits faster, determine how much money you receive, or decide when your disability funds end.

Need an Extension?

It is possible for your disability to get extended past the original date indicated on your EDD claim. If you need an extension on your disability, please contract our office with your EDD receipt number.

Private Insurance or FMLA Benefits

First Time?

  1. Ask your employer (HR) about disability benefits and form(s) to use.
  2. Fill out your part of the forms.
  3. Fax, email, or request your disability form(s) to be sent to your provider. Please note that our office will need time to complete the form(s) and we do charge a $30 fee per form.
  4. Your Employer/Disability Insurance Company will review your form(s) and give you disability benefits.

Need an Extension?

It is possible for your disability to get extended past the original date indicated. If you need an extension on your disability, please provide our office with additional forms. Please note that our office will need time to complete the form(s) and we do charge a $30 fee per form.

*These forms may be completed in the OSI office, after your appointment, at check out. It may take 20 minutes to complete. Some forms may be completed during clinic hours if time is not permitting.

OSI does not process long-term disability benefits with the state of California. If you need to apply for long-term benefits from the government, please contact the local Social Security Office in California.

Medical Records

The request to obtain medical records does not require approval from a doctor. Just let our staff know to help you. You will still be responsible for the administrative fee.

Orthopaedic Specialty Institute, LLC outsources our release of information process to HealthMark Group. To follow up on the status of your medical record request, you may contact HealthMark Group directly at 800-659-4035 or [javascript protected email address] .

HealthMark Group fulfills all patient requests for personal copies at no charge to the patient. A complimentary copy of your record will be made available through HealthMark Group’s website, www.healthmark-group.com under Manage Your Request. First time users will no longer need to setup an account, and only need to provide an email address to get started.

You may submit a request electronically with HealthMark Group by visiting requestmanager.healthmark-group.com. Once logged in, you may select Submit Request from the menu options and enter all required fields to submit an authorization to HealthMark Group directly. HealthMark Group will process your medical record request and provide notification via mail or email once complete. A complimentary copy of your record will be made available for you to download through HealthMark Group’s Request Manager website.
